At Bellbrooke Surgery, we help patients with asthma and COPD through a yearly health check. At this appointment, we check your breathing, make sure your inhaler is right for you, and give advice to help you stay well.

Taking Care of Your Health

Looking after your health is a team effort. We are here to support you, but you should also take care of yourself. This means:

  1. Using your inhaler correctly and regularly
  2. Knowing when your breathing is getting worse
  3. Asking for help before it becomes an emergency

By doing this, you can avoid hospital visits and feel better every day.

Dementia services

Memory problems

It is very important that anyone who has problems with their memory has a proper assessment. If these problems are due to dementia then early diagnosis and referral have many benefits for the patient, their carer(s) and family. It can help them to plan and access the treatment and support they need and to improve their quality of life.  You can book an appointment with one of our Doctors who can make the initial assessment and refer you to the memory clinic if needs be.

We will invite you for an annual health check when you are due to attend.  This will initially take place with one of our healthcare assistants who will also arrange for you to be reviewed by one of our Doctors.  If you have any other conditions we will review these at the same time.

Annual Health Check

If you have one of the following conditions then you will be invited for an annual health check.

  • Diabetes
  • High blood pressure
  • Kidney disease
  • Heart disease
  • Asthma
  • Chronic bronchitis
  • Rheumatoid arthritis
  • Learning disabilities
  • Dementia
  • Severe mental health problems

We invite all people for their annual health check around the same month each year.  You may need a blood test before the appointment, in which case the invite will say this.

Diabetes Services

There are a number of services that can help you when it comes to looking after yourself if you have diabetes.

Routine care

The majority of people with diabetes are looked after by the practice diabetes team.

We like to see all people with diabetes twice a year, once during their annual health check and again six months later.  If you have other long term conditions, we will review these at the same time as your diabetes.

You will receive an invite for both the annual health check and the six month review. Our health care assistants will see you first to collect some blood tests and work through a set of health checks before you see a practice nurse.  In some cases, the practice nurse may arrange an appointment to see one of our General Practitioners (GPs).

There are times when you will need to be seen more often than twice a year, for example after you are first diagnosed or if problems are detected during one of the routine reviews. We arrange these additional reviews as needed and may be a nurse or one of the Doctors.

The practice team

There are three practice nurses in the team Caroline, Helen and Lucy and a nurse associate, Nicola.

The diabetes lead for the practice is Dr Vidush Gautam and is supported by Helen the Lead Diabetes nurse.

Specialist care

If there is a particular problem identified with your diabetes care that we cannot help you within the practice, then we will refer you to a specialist service.

For the majority of specialist problems we use Leeds Community Healthcare diabetes service, though for some people we also refer to St. James’s Hospital.

Self help and support groups

There are 8,760 hours in a year and over a year you will only spend two to three hours receiving support and advice about your diabetes from professionals. The vast majority of care comes from ‘self care’.

We suggest that all people who develop diabetes should attend the self-management education programme, which is 6 hours of extra education offered to those with a new diagnosis of diabetes (your nurse will mention it).

Child Health

Health visitors

Health Visitors are all trained nurses who have extra training and qualifications in child health. They are a fantastic source of information about child development and health, parenting strategies, breastfeeding, nutrition and community support. They also offer help with social, emotional and environmental issues that affect families.

Child health checks

Appointments are sent out automatically for routine child health checks.

The first will be with a GP at the surgery when your baby is around 6 weeks old.You will be sent information about this and asked to make an appointment at the practice.

You will also be offered routine health reviews for your child, with your health visitor, at 6-8 weeks, 8-12 months and 2-2½ years old.

Immunisations are given by our practice nurses in accordance with the NHS vaccine schedule.

Weight Management

Do you need to lose weight? There are a number of online resources and local clinics that you can access without having to see your doctor for a referral.

Advice for adults:

NHS Choices weight loss guide is an excellent guide with suggestions on a 12 week plan. It also has email support.

Face to face support

Support for adults:

  • One You Leeds offers a range of free NHS service to help you loose weight. This is the service that our Nurses and Doctors will refer you to if you need additional support. You can self refer to this service.
  • There are also private services (that you would need to pay for) such as Weight Watchers and Slimming World who both provide popular classes locally.

NHS Health Check

The NHS health check is a free health check-up for adults in England aged 40-74.

It aims to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes and dementia because as we get older our risk of developing one of these conditions increases.

If you would like to book an NHS health check please contact reception to make an appointment.

The health check only takes about 30 minutes.

The nurse/health professional will ask you some simple questions about your lifestyle and family history, measure your height and weight, take your blood pressure and do a blood test. From this, they can give you an idea of your risks. If you are over 65, you will also be told the signs and symptoms of dementia to look out for.

You will then receive personalised advice to help lower your risk. This could include:

  • How to improve your diet and the amount of physical activity you do
  • How to lose weight or stop smoking

If a particular problem is found, such has high blood pressure, then we will make an appointment for you to see a doctor.

Sexual Health Service

The practice offers a full range of contraception services including coil fitting and referral for vasectomy.
