Practice Area

You can register as a new patient with the practice if you live in our inner practice area. You can check this using the tool below.

If you are registered in our inner practice area and then move to our outer practice area you may remain on our list.

How to register as a new patient

You may send us a request to Register via this online registration form. We will process this registration request within 5 working days. Once we have processed your request to register, you will receive an automated confirmation if this has been successful via the NHS app, email, SMS text or letter. If we require any further information to support your request to register or cannot accept your registration, we will contact you directly.

Visit the surgery (between 12:00pm and 6:00pm) in person and complete an application form at reception. When you return your registration form, we will check that you are within our boundary this and process it within 5 working days. If we require any further information to support your request to register, we will contact you directly.

You can also download and print the paper registration form yourself and bring them in to the practice.

New patients may be asked to complete an initial health questionnaire following which you will be offered a preliminary health check.

Overseas Visitors

If you are an overseas visitor to the UK there may be a charge to see the doctor or nurse. For further information, please contact reception.

Updating Your Details

Please let us know if you move address, change your phone number(s) or email. You can do this in person or if you are registered for online services you can do this via the patient portal.

Temporary residents

If you are visiting the area for work, family reasons, holiday or studies, you can register temporarily (from 16 days to 3 months) with Bellbrooke Surgery. You’ll still remain registered with your permanent GP back at home.

To register as a temporary resident the place you are staying needs to be within our inner catchment area. After three months you will have to apply to register with that surgery as a permanent resident.

We will pass details of any treatment you have to your GP, who will add the information to your medical records.
